Operational safety is not just about compliance with regulations. It involves daily decisions, the way technical teams operate, and an organizational culture that recognizes risks before incidents occur. In many companies, safety remains the domain of the HSE department, while operational leaders lack the necessary training to effectively support it in practice.

 

The training "Operational Safety in Practice" is an intensive two-day program that develops key competencies for technical leaders, engineers, maintenance specialists, and HSE professionals. Participants learn how to make decisions under uncertainty, how to analyze operational risk from a technical team's perspective, and how to build habits that genuinely impact safety—without excessive bureaucracy.

  • Examples of decision-making errors that led to incidents
  • Workshop: map of critical situations – what could go wrong?

 

Module 2: Risk Assessment and Permit to Work – Process Approach

 

  • Risk assessment in practice – from checklists to contextual decisions
  • Risk matrices, dynamic hazard assessment, context analysis
  • Permit to Work – logic, applications, common errors
  • Workshop: preparation of PTW and dynamic risk assessment for a complex task

 

Module 3: Accident Investigation and Root Cause Analysis

 

  • Overview of RCA methods (5 Why, Tripod Beta, Apollo, fishbone)
  • Swiss Cheese Model – layers of defense and their perforations
  • Bow-Tie Method – causes, consequences, barriers
  • Workshop: analysis of a real incident – Swiss Cheese model + Bow-Tie

 

Module 4: Hazardous Work and Non-standard Actions – How to Manage

 

  • Definition and classification of particularly hazardous work
  • Management of operational change (MOC) – when required, how to apply it
  • Principles of escalation, communication, and documentation of non-standard actions
  • Workshop: analysis of a "non-standard" task – how to approach risk assessment

 

Module 5: Crisis Management and Leadership in Difficult Situations

 

  • How people react under pressure – psychology and decision-making errors
  • The role of the technical leader in a crisis – competencies vs. formal functions
  • Communication, command, rapid situation assessment
  • Workshop: simulation of a crisis event – real-time decision-making

 

Module 6: Safety Culture – How to Build and Strengthen It
